What to Bring to Your First Appointment
To help us provide you with the best possible care from the start, please bring a valid photo ID and your insurance card, a list of any current medications including vitamins and supplements, any relevant previous medical records or test results, a list of questions or concerns you’d like to discuss, and your completed new patient forms if available in advance.
